Abstract: Introduction: Acute myocardial infarction has a high prevalence and possibility of death if timely care is not given. It is possible that treatment could be delayed during a health crisis such as the COVID-19 pandemic. Objective: The objective of this study was to evaluate the impact of the COVID-19 pandemic on the function of the infarction code in the High Specialty Medical Unit of our institution. The medical indicators of acute coronary ischemic syndrome with ST-segment elevation were compared at two times: before the COVID-19 pandemic and during it. Material and methods: An ambispective, analytical, comparative cohort study was conducted, comparing the periods of February-September 2019 and February-September 2020, in the High Specialty Medical Unit, Centro Médico del Bajío number 1, Instituto Mexicano del Seguro Social, Leon, Guanajuato, Mexico. All patients older than 18 years with clinical, electrocardiographic, and enzymatic data of acute coronary ischemic syndrome, accepted in the medical network as infarction code registered in the National Registry of Acute Coronary Ischemic Syndromes (RENASCA) electronic case report form were included in the study. Results: The independent samples t-test was used to determine the impact of the infarction code initiative based on the reperfusion therapies performed, the door-to-needle, the door-to-balloon time, and the ischemia time. There was a significant difference in the reperfusion strategy, with greater thrombolysis during the COVID-19 pandemic (57.4% versus 72.6%, odds ratio [OR] 1.97, 95% confidence interval [CI] 0.49-0.82, p < 0.001), as well as reperfusion criteria (21.03% versus 35.37%, OR 2.05, 95% CI 0.53-0.80, p < 0.0001). Mortality was not different between the time periods. Conclusions: The COVID-19 pandemic has not impacted operation of the infarction code in the High Specialty Medical Unit Centro Médico del Bajío number 1 because the medical indicators of acute coronary ischemic syndrome with ST-segment elevation did not change.

ABSTRACT Background: Information is needed on the safety and efficacy of direct discharge from the emergency department (ED) of patients with COVID-19 pneumonia. Objectives: The objectives of the study were to study the variables associated with discharge from the ED in patients presenting with COVID-19 pneumonia, and study ED revisits related to COVID-19 at 30 days (EDR30d). Methods: Multicenter study of the SIESTA cohort including 1198 randomly selected COVID patients in 61 EDs of Spanish medical centers from March 1, 2020, to April 30, 2020. We collected baseline and related characteristics of the acute episode and calculated the adjusted odds ratios (aOR) for ED discharge. In addition, we analyzed the variables related to EDR30d in discharged patients. Results: We analyzed 859 patients presenting with COVID-19 pneumonia, 84 (9.8%) of whom were discharged from the ED. The variables independently associated with discharge were being a woman (aOR 1.890; 95%CI 1.176-3.037), age < 60 years (aOR 2.324; 95%CI 1.353-3.990), and lymphocyte count > 1200/mm3 (aOR 4.667; 95%CI 1.045-20.839). The EDR30d of the ED discharged group was 40.0%, being lower in women (aOR 0.368; 95%CI 0.142-0.953). A total of 130 hospitalized patients died (16.8%) as did two in the group discharged from the ED (2.4%) (OR 0.121; 95%CI 0.029-0.498). Conclusion: Discharge from the ED in patients with COVID-19 pneumonia was infrequent and was associated with few variables of the episode. The EDR30d was high, albeit with a low mortality.

ABSTRACT The outbreak caused by the SARS-CoV-2 virus is currently very active in Spain. Many infected people still require to be hospitalized. Around 10-15 % of hospitalized patients require intensive care, where they are intubated for a prolonged period, needing tracheotomies some weeks after the intubation. We will be conducting an observational study of the tracheotomies performed by our oral and maxillofacial Department to COVID-19 patients on intensive care units between March 17th and April 17th, 2020. This study will be analyzing the patients' epidemiological and clinical aspects, surgical technique employed, surgical time, type of cannula used, postoperative complications and the patients' clinical monitoring. A total of 22 patients underwent open elective tracheotomy. There were twenty-two males and three females aged between 40 and 77 (mean: 64,9 years-old). In all cases tracheotomy was carried out due to pulmonary process caused by COVID-19 bilateral pneumonia. Two patients presented pneumothorax in the immediate postoperatory care as a complication, one perished during the procedure and another did so after arriving to the Intensive Care Unit after the tracheotomy surgery. Even though tracheotomy is a ruled surgical technique, the special characteristics of COVID-19 patients make of this procedure a critical situation, mainly due to lung instability and quick desaturation of the patients. This requires the surgery to be carried out by experienced physicians in order to reduce operative time and to be able to react to any eventualities that may arise.
RESUMEN El brote epidémico causado por el virus SARS-CoV-2 se encuentra plenamente activo en España. Alrededor del 10-15 % de los pacientes ingresados precisan cuidados en unidades de críticos, siendo intubados de forma prolongada y precisando la realización de traqueotomías. Se realiza un estudio observacional de las traqueotomías realizadas por nuestro Servicio de Cirugía Oral y Maxilofacial a pacientes COVID-19 de unidades de cuidados intensivos realizadas entre el 17 de marzo y el 17 de abril de 2020. El estudio analiza aspectos epidemiológicos y clínicos de los pacientes, el tipo de técnica quirúrgica empleada, el tiempo quirúrgico, el tipo de cánula empleada, las complicaciones postquirúrgicas y el seguimiento clínico de los pacientes. Un total de 22 pacientes fueron sometidos a traquetomía reglada abierta. Fueron dieciocho hombres y cuatro mujeres de edades entre 40 y 77 años (64,9 años de media). En todos los casos la realización de traqueotomía fue como consecuencia del proceso pulmonar por la neumonia bilateral COVID-19. Dos pacientes presentaron un neumotórax en el postoperatorio inmediato como complicación, un paciente falleció durante la realización del procedimiento y otro tras su llegada a la Unidad de Cuidados Intensivos tras la realización de la traqueotomía. A pesar de que la traqueotomía es una técnica quirúrgica reglada, las características especiales de los pacientes COVID-19 hacen de este procedimiento una situación crítica por la inestabilidad pulmonar y la rápida desaturación del paciente. Todo ello obliga a la realización del procedimiento por facultativos con experiencia para disminuir el tiempo quirúrgico y poder enfrentarse a cualquier eventualidad.

Abstract The recommendations in which the Mexican Society of Cardiology (SMC) in conjunction with the National Association of Cardiologists of Mexico (ANCAM) as well as different Mexican medical associations linked to cardiology are presented, after a comprehensive and consensual review and analysis of the topics related to cardiovascular diseases in the COVID-19 pandemic. Scientific positions are analyzed and responsible recommendations on general measures are given to patients, with personal care, healthy eating, regular physical activity, actions in case of cardio-respiratory arrest, protection of the patient and health personnel as well as precise indications in the use of non-invasive cardiovascular imaging, prescription of medications, care in specific topics such as systemic arterial hypertension, heart failure, arrhythmias and acute coronary syndromes, in addition to emphasizing electrophysiology, interventionism, cardiac surgery and in cardiac rehabilitation. The main interest is to provide the medical community with a general orientation on what to do in daily practice and patients with cardiovascular diseases in the setting of this unprecedented epidemiological crisis of COVID-19.
Resumen Se presentan las recomendaciones en las cuales la Sociedad Mexicana de Cardiología (SMC) en conjunto con la Asociación Nacional de Cardiólogos de México (ANCAM), así como diferentes asociaciones médicas mexicanas vinculadas con la cardiología, después de una revisión y análisis exhaustivo y consensuado sobre los tópicos relacionados con las enfermedades cardiovasculares en la pandemia de COVID-19, se analizan posturas científicas y se dan recomendaciones responsables sobre medidas generales a los pacientes, con cuidados personales, alimentación saludable, actividad física regular, acciones en caso de paro cardiorrespiratorio, la protección del paciente y del personal de salud así como las indicaciones precisas en el uso de la imagen cardiovascular no invasiva, la prescripción de medicamentos, cuidados en tópicos específicos como en la hipertensión arterial sistémica, insuficiencia cardiaca, arritmias y síndromes coronarios agudos, además de hacer énfasis en los procedimientos de electrofisiología, intervencionismo, cirugía cardiaca y en la rehabilitación cardiaca. El interés principal es brindar a la comunidad médica una orientación general sobre el quehacer en la práctica cotidiana y pacientes con enfermedades cardiovasculares en el escenario esta crisis epidemiológica sin precedentes de COVID-19.

Abstract: Objective: To assess the performance of medical schools (FEM) by analyzing the results of their applicants in the Examen Nacional para Aspirantes a Residencias Médicas (ENARM). Materials and methods: Five performance criteria, two official and three created on purpose, were calculated from the ENARM-2016 and -2017 official databases to assess FEM performance. Results: In 2016 and 2017, applicants registered from 112 and 115 FEM, respectively. Depending on the performance criteria, the FEM in the first place obtained 5 to 20 points more than the one placed second, and 23 to 98 points more than the FEM in the last place. Approximately 25% applicants were classified as “knowledge-deficient,” and about 80% of these originated from less than one third of the FEM. Conclusion: The ENARM results provide information on the performance of the FEM. Approximately one of every four applicants obtained scores lower than the approval threshold of any specialty.
Resumen: Objetivo: Evaluar el desempeño de las facultades y escuelas de medicina (FEM) utilizando como subrogado los resultados del Examen Nacional para Aspirantes a Residencias Médicas (ENARM). Material y métodos: Se analizaron las bases de datos oficiales del ENARM 2016 y 2017, empleando cinco criterios de desempeño (CD) por cada FEM: dos oficiales y tres creados exprofeso. Resultados. En 2016 y 2017 se registraron sustentantes de 112 y 115 FEM, respectivamente. Dependiendo del CD, la FEM que quedó clasificada en el primer lugar obtuvo entre 5 y 20 puntos más que la del segundo lugar, y entre 23 y 98 puntos más que la FEM ubicada en el último lugar. Aproximadamente 25% de los sustentantes fueron calificados como “deficientes en conocimientos” y aproximadamente 80% de éstos provenían de menos de un tercio de las FEM. Conclusiones: El ENARM arroja información sobre el desempeño de las FEM. Aproximadamente uno de cada cuatro sustentantes obtuvo puntajes menores al aprobatorio en cualquier especialidad.

Abstract Objective: To present our experience in relation to the safety and efficacy of the 180W Greenlight XPS laser for photoselective vaporization of the prostate. Materials and methods: A retrospective, observational, analytic, and descriptive study was conducted within the time frame of July 2014 to November 2017, evaluating technical aspects and preoperative and postoperative variables at 1, 3, and 6 months. Results: Fifty-five patients were included in the study. Their mean age was 69 years. A total of 49.1% presented with comorbidities, 14.5% with heart disease, and 20% with an ASA III. The preoperative parameters were: IPSS 14.5 points (5-35), Qmax 7.19 ml/sec (1.5-12), prostate volume 74 cc (29.2-130), PSA 5.7 ng/ml, and Foley catheter placement (54%). Laser use, time with catheter, previous infections, and previous biopsy were described, as were intraoperative complications (10.8%), postoperative complications (27.3%), Clavien-Dindo (C-D) I (80.2%), C-D II (6.6%), and C-D IIIa (13.2%). Postoperative control at 6 months was: Qmax 17.2 ml/sec (10.2-34.4) (p=0.01) and IPSS 4.2 points (0-19) (p=0.02). Surgical reintervention was performed in 3.6% of the patients, hospital stay was 1.5 days, and the mortality rate was 0%. Conclusions: Photoselective vaporization of the prostate with the GreenLight laser is a safe surgical technique with good results in high-risk surgical patient populations. Complications are similar to those when utilizing the gold standard, but patients taking anticoagulants and antiaggregants can be operated on with the technique and hospital stay is short. Studies with long-term follow-up are needed.
Resumen Objetivo: Presentar experiencia en seguridad y eficacia con el láser GreenLight XPS de 180w para fotovaporización selectiva de la próstata (FVP). Materiales y Métodos: Se realizó un estudio retrospectivo, observacional, analítico y descriptivo (de julio de 2014 a noviembre de 2017). Se evaluaron aspectos técnicos, así como variables pre y posquirúrgicas al primer mes, tercero y sexto. Resultados: 55 pacientes, media de 69 años, 49.1% con comorbilidades, 14.5% cardiopatías, asaiii 20%. Parámetros prequirúrgicos: ipss 14.5(5-35)pts, Qmax 7.19(1.5-12)ml/seg, volumen prostático 74(29.2-130)cc, ape de 5.7ng/ml, 54% portadores de sonda foley. Se describen parámetros de uso de láser, tiempo de sonda, infecciones previas y biopsia previa, complicaciones transoperatorias 10.8% y postoperatorias 27.3%, Clavien-Dindo (CV) I en 80.2%, cvii 6.6% y cviiia 13.2%. Controles postquirúrgicos 6 meses: Qmax 17.2(10.2-34.4)ml/seg (p=0.01) e ipss 4.2(0-19)pts (p=0.02). Reintervención quirúrgica 3.6%, 1.5 días de estancia intrahospitalaria y una mortalidad de 0%. Conclusiones: La cirugía fvp con GreenLight es una técnica quirúrgica segura, con buenos resultados en poblaciones de pacientes con alto riesgo quirúrgico, las complicaciones no son mayores que el estándar de oro, pero los pacientes con anticoagulantes y antiagregantes pueden ser operados con esta técnica, con poco tiempo de hospitalización. Se requiere seguimientos a largo plazo.

Abstract: Objective: To assess the assumption of ‘equity’ of Mexico’s resident-selection assessment tool, the Examen Nacional para Aspirantes a Residencias Médicas (ENARM). Materials and methods: Official ENARM-2016 and -2017 databases were analyzed. Differences in the absolute number of correct answers (multivariable linear regression) and the number of applicants reaching their specialty minimum score (SMS) per test day (odds ratio [OR]) were calculated. Applicants affected by test-day inequity were estimated. Results: There were 36 114 applicants in 2016, and 38 380 in 2017. In 2016, day-2 applicants had significantly higher scores and more reached the SMS than on days 1-3-4 (OR 1.55), and 5 (OR 3.8); 3 565 non-passing applicants were affected by inequity (equivalent to 44.64% of those selected). In 2017, day-1 and -2 applicants had significantly higher scores and more reached the SMS than on days 3-4 (OR 1.85), and 5 (OR 4.04); 3,155 non-passing applicants were affected by inequity (37.2% of those selected). Conclusion: Analysis of official ENARM databases does not support the official attribution of equity, suggesting the test should be redesigned.
Resumen_ Objetivo: Evaluar el atributo de “equidad” asignado al Examen Nacional para Aspirantes a Residencias Médicas (ENARM). Material y métodos: Se analizaron las bases de datos oficiales del ENARM 2016 y 2017. Se compararon las diferencias inter-día de respuestas correctas (regresión linear multivariable) y de sustentantes que alcanzaron el puntaje mínimo de su especialidad (PME) (razón de momios [RM]). Se estimó a los afectados por la inequidad. Resultados: Hubo 36 114 sustentantes en 2016 y 38 380 en 2017. Los días 2 (ENARM-2016) y 1-2 (ENARM-2017) registraron puntajes significativamente más altos, y más sustentantes alcanzaron el PME que en los días 1-3-4 (RM 1.55) y 5 (RM 3.8) en 2016, y los días 3-4 (RM 1.85) y 5 (RM 4.04) en 2017. Se estimó que cuatro de cada diez sustentantes que aprobaron el ENARM no lo hubieran hecho si el examen fuera equitativo. Conclusión: Los resultados sugieren que el atributo de equidad del ENARM está en duda.

Abstract Background: Cancer of the penis accounts for less than 1% of all cancers affecting males. Nevertheless, it has a huge impact on the patients that present with it. Objective: To report on the quality of life, erectile function, and perception of self-esteem in patients that underwent partial phallectomy due to cancer of the penis. Materials and methods: An analytic, cross-sectional study was conducted that included 10 postoperative partial phallectomy patients. They were evaluated through the EORTC-QLQ-30 questionnaire (validated for the Mexican population), the International Index of Erectile Function-Short Form (IIEF-5), and the Rosenberg Self-Esteem Scale (SES). Results: The quality of life results, evaluated through the EORTC-QLQ-30 questionnaire, were lower than those for the general population in all 10 patients. In the IIEF-5 sexual activity evaluations, 5 of the 10 patients were sexually inactive, one patient had no deterioration of sexual activity, one patient had mild deterioration, and 3 patients had mild-to-moderate deterioration. Self-esteem, evaluated by the SES, was above average in 8 of the 10 patients, average in one patient, and below average in one patient. Conclusions: The results of the present study, determined through different scales, indicated that partial phallectomy as treatment for cancer of the penis affected patient quality of life and had repercussions on sexual function. Nevertheless, the self-esteem of the patients, despite their condition, was not affected and was even above average.
Resumen Introducción: El cáncer de pene representa menos del 1% del total de cánceres que afectan al género masculino; sin embargo, implica un gran impacto para el paciente por lo que esto representa. Objetivo: Reportar la calidad de vida, función eréctil y percepción de autoestima de los pacientes sometidos a falectomía parcial por cáncer de pene. Materiales y métodos: Estudio analítico de cohorte transversal. A 10 pacientes posoperados se les aplicó el cuestionario EORTC-QLQ-30 (validado para población mexicana), Índice Internacional de Función Eréctil 5 y SES (Cuestionario de Autoestima de Rosenberg). Resultados: De los 10 pacientes, la calidad de vida, evaluada con el cuestionario EORTC-QLQ-30, fue menor que los resultados de la población general; la actividad sexual, evaluada por la Escala IIEF-5, 5 de 10 pacientes eran sexualmente inactivos, 1 no tuvo deterioro, 1 tuvo un deterioro leve y 3 un deterioro leve-moderado; el autoestima, evaluada por la Escala de Autoestima de Rosenberg, 8 de 10 pacientes presentaron un autoestima arriba del promedio, 1 dentro del promedio y 1 baja autoestima. Conclusiones: Los resultados obtenidos, evaluados según las distintas escalas, indican que la Falectomía Parcial como tratamiento de Cáncer de Pene, afecta la calidad de vida y puede tener repercusiones respecto a la función sexual, sin embargo, la autoestima, pese a la condición de los pacientes, se muestra sin afección, incluso, mejor que el promedio.

Abstract: Introduction: Gastrointestinal stromal tumors (GIST) represent a group of non-epithelial rare tumors with an estimated incidence of 10 to 20 per million persons. GISTs can present at any age but more commonly in persons between ages of 40 and 70 years. They arise more frequently from the stomach and have Kit gene mutation. Material and methods: We presented a case of a 39-year old male with a giant abdominal tumor with surgical resection and review of the literature of GIST. Results: A wedge resection of a GIST measuring 49 × 45 × 15 cm was performed. Microscopically, it was a spindle-shaped tumor cells GIST, low-grade histology (1), pT4 (more than 10 cm), with an intermediate risk (mitotic count less than 5 per 50 HPF and more than 10 cm) and complete resection (R0). Conclusion: In this case, we describe the clinical presentation and the diagnosis algorithm of a low grade giant gastric GIST. To our knowledge, this could represent the biggest GIST reported in the literature.
Resumen: Introducción: Los tumores del estroma gastrointestinal (GIST) son tumores no epiteliales poco frecuentes con incidencia de 10-20 casos por millón de habitantes; se presenta en personas de 50 a 71 años de edad. Se presentan principalmente en estómago y tienen como característica mutaciones en el gen Kit. Métodos: Se describe el cuadro clínico de un paciente masculino de 39 años que presentó un tumor abdominal gigante con tratamiento quirúrgico y realizamos la revisión bibliográfica de los GIST. Resultados: Resección quirúrgica de un tumor del estroma gastrointestinal de bajo grado con dimensiones de 49 × 45 × 15 cm. Desde el punto de vista histopatológico se trató de un GIST de células fusiformes bien diferenciado, pT4 (de más de 10 cm), de riesgo intermedio (índice mitótico menor a 5 mitosis por 50 campos y mayor a 10 cm) y resección completa (R0). Conclusiones: En este caso describimos la presentación clínica y el protocolo de estudio de un GIST gástrico gigante de bajo grado. Este tumor podría representar el GIST más grande reportado en la literatura según nuestro conocimiento.

Concentración de almidón y proteínas solubles en tubérculos de Caladium bicolor en diferentes etapas fenológicas

In this paper the concentration of starch and soluble proteins (mg g-1 DM) in tubers of Caladium bicolor was evaluated through the application of organic products (humic and fulvic acids) diluted in irrigation water. Treatments consisted in the interaction of three doses of organic product (0 (control), 4 and 8 ml L-1) and three phenological stages of the crop (flowering, predormancy and dormancy. The variables were measured based on UV-visible spectrophotometry, using mathematical models derived from calibration curves of each organic compound. The data was analyzed under a completely randomized design with a factorial arrange 3*3. The treatments with 8 and 4 ml L-1 of organic product (humic and fulvic acids) on dormancy stage showed higher concentration of starch with 885.18 and 833.97 mg g-1 DM respectively. Furthermore, the highest concentration of soluble proteins in treatment of 4 and 8 ml L-1 was obtained at flowering stage with 156.04 and 150.54 mg g-1 DM respectively. Under the conditions in which this research was conducted, it is concluded that the application of organic products generates greater concentration of vegetative storage in tubers of this crop, which will serve to produce bigger and more shoots for the next production cycle.
En el presente trabajo se evaluó la concentración de almidón y proteínas solubles (mg g-1 de MS) en tubérculos de Caladium bicolor mediante la aplicación de productos orgánicos (ácidos húmicos y fúlvicos) diluidos en el agua de riego. Los tratamientos consistieron en la interacción de tres dosis de producto orgánico (0 (testigo), 4 y 8 ml L-1) y tres etapas fenológicas del cultivo (floración, predormancia y dormancia. Las variables fueron medidas en base a la técnica de espectrofotometría UV-visible, mediante modelos matemáticos derivados de curvas de calibración para cada compuesto orgánico. Los datos obtenidos fueron analizados bajo un diseño completamente al azar con arreglo factorial 3*3. Los tratamientos con 8 y 4 ml L-1 de producto orgánico (ácidos húmicos y fúlvicos) en la etapa de dormancia, presentaron mayor concentración de almidón con 885.18 y 833.97 mg g-1 de MS respectivamente. Por otra parte, se obtuvo la mayor concentración de proteínas solubles en los tratamientos de 4 y 8 ml L-1 en la etapa de floración con 156.04 y 150.54 mg g-1 de MS respectivamente. Bajo las condiciones en las que se desarrolló la presente investigación, se concluye que la aplicación de productos orgánicos genera mayor concentración de reservas vegetativas en el tubérculo de este cultivo, lo cual servirá para que produzca brotes en mayor tamaño y cantidad para el siguiente ciclo de producción.
